Truck is a 1999 with a 4.0.  First noticed it yesterday.  Fired the truck up and immediately dropped it into reverse.  It shook like it was missing.  Back it up out oo my way and parked it.  Later that day I went too move it again and same thing.  This morning I went to run errands and I decided to let it run for a minute.  Upon start up the truck had a miss then it cleaned up.  Ran around a bit and when I parked I fired it back up and no miss.  So it's only when cold

160k on the truck.  Less than 10k on plugs, wires, iac, air and fuel filters.  No codes.  Thoughts?

Ugh too many things it could be. I would look at the wires and make sure one of them didn't migrate to the exhaust manifold, could be an injector but you would think it would throw a code.

I had a head gasket leak in a dodge, between a coolant port and a cylinder. The radiator cap was relieving the pressure during running but when it would sit over night the coolant would seep in to the cylinder. Would miss at startup when cold every time. Worse case scenario and difficult to detect without one of those "exhaust gasses in the coolant" test kits. One of our better mechanics in the back (who quit later) suspected this and ordered a kit which proved positive.

I looked at the simple stuff. Wires look fine. Connections are good. Trucks not using any coolant. And I really don't have time for a head gasket.

I've got a local Indy shop that might be able to test it for me

Dusten, 1st thing I would do is pull all the plugs & check them. A little while after I purchased my 2000 Ranger 4.0 in 2013 it developed a miss, the plug in the driver's side front cylinder would foul out. I would replace the plug & it would be fine for a couple months, then it would foul out again. It turns out the heads were cracked between the valves front cylinder drivers side head, & rear cylinder passengers side head. I replaced both cylinder heads with aftermarket heads that had more metal between the valves so hopefully this doesn't happen again.

I'm going to pull the plugs tomorrow or the next day. I'll probably drain the oil. I'll check compression too. I found some complete heads for $200 each. Head bolts and a head change kit are $200. Belts, hoses, plugs Add another $50 for fluids and I'll be about 600-700 a head swap. Figure I can do it in a day or two.

Took it for a run to the next town of to pick up a new sink.  Logged coolant temps.  They hover between 188-196 which seems normal.  This was freeway in town and idling.

On "the dodge" there was no detectable coolant loss and temps were normal. Compression was good too.

This is a known problem on the 4.0 OHV engines.  A very small leak can develop in the intake gasket which allows a tiny amount of coolant to be drawn into the intake runner. This is just enough to cause a misfire but not enough to show up as a noticeable coolant loss from the reservoir. An indicator is a very different plug reading on the affected cylinder, most commonly seen on cylinder #4. Could also be cracked head as noted above but this is worth checking too.

Try retorquing the lower intake bolts, or the upper to lower  rubber intake seals could be leaking, they are hard rubber & over time get crushed & leak. In 2015 I had a miss on a cold engine on my 4.0 & replaced them & retorqued the lower intake (it was loose) & it solved the problem. The upper intake seals are the 3 on top in the link pic.

I have a valve cover, upper intake gasket set. I could replace both and check the torque I'll going to pull the plugs first

It probably is the intake, when I had the cold start miss the upper to lower intake seals & retorquing the lower intake fixed the problem for me. When I had the cracked heads the miss would happen whether the engine was cold or hot & he is only complaining about it with a cold engine.

I'm not pulling a head. If after I do the intake it's still an have the head looked at will be close enough to a reman head that i'd rather go that route.

I'm leaving for virginia in less than a month. I dont have time to be tearing the motor down for no reason.

I found the thread about the coolant leak. Looks like this was in the lower intake? I'm pulling everything off but I have both upper and lower intakes spare but like an idiot sold the heads I had. Bet I can't find a pair for 50 bucks which is what I sold them for.

Ya it was the lower intake.  I found coolant on the plug and signs of a fouled gasket.  Zero issues after.  Trucks been gone a while though
