Is that an aftermarket/different than stock shift knob? If so, how did you get the old one off?! Thanks

I'm not sure about 98+ but the 83-97 it's best to take a wrench that slips around the shifter itself, and tap upward on it with a rubber mallet and it pops off. There are splines on the end much like you'd expect on a ball joint.

So what's with the transfer case lever.... I didn't think any ranger had those is that a level 2 accessory also?

The knobs come stock on the truck. Mine just screws on..has a lock nut under the ball

Yep, stock manual transfer. I believe the 2002 fx4 and the 03 fx4 level 2 came with the "twin sticks"
