2004 Ranger XLT FX4 4.0 automatic.  The transfer case is a BW1354 electric shift.

Has anyone ever seen this and if so, what caused it?

I have never in my life seen one crack. I would bet you ran over something that didn't seem like a big deal, a 2 X 4 in the road, crowbar, concrete...

Unless you just bought that particular truck and it was damaged by improper lifting on a hoist, someone moved it with a fork lift (I've seen this break a U joint) prior to you buying it.

It would be interesting to hear how you discovered this, how long you had the truck, stuff like that.

I've had the truck for a bit over five years.  I recently blew low range out climbing a pretty steep hill.  We got this transfer case from a wrecking yard.  It had no visible cracks or blemishes.  We swapped it out and replaced all three u-joints.  The first indication of trouble was a sound like putting a car in park while still rolling when coasting to a stop.  We could ride the brakes a little and it went away.  We discovered the cracks when we considered disconnecting the transfer case motor.

That's the problem with wrecking yards (we call them scrapyards) - you have no idea of the history of the unit.
