I don't think I have seen the long term fuel trim that high. Ever. You poor guy!

As far as fixing, one thing at a time.

If it is a vacuum leak it could be as easy as the hose to the power brakes and as deep as intake manifold gasket. If it will stay running you need to listen under the hood, I use a poor mans stethoscope.. a piece of small hose in my ear placing the other end at spots I suspect there might be leaks. Too bad you don't live up the street I'd come by and take a look.
