Just a wild guess based on an experience I just had on my 2.5. These engines at high miles can have recessed valve seats. When they recess (or actually wear away), they thin the head castings too much and crack so you have to replace them.

Now a way for valve recession to accelerate is to have high head temps/exhaust temps and that can be caused by excessive back pressure from a restrictive catalytic converter. 

It is possible they replaced the head for recessed valves, but never fixed the original causing problem of a clogged/restrictive cat.

Not saying at all this is the case, but logically it can fit the issue.
