Providing everything is working right on your gun the charge is not strong enough to send the slide fully back to lock into position. One of the most common issues of a slide not locking open on the last round is under-powered ammo. Winchester White Box is notorious for being loaded for light loads as well as some other popular "target" ammo. Get some NATO rated ammo and see if it locks the rear slide open. The NATO specs are at the upper end of the power spectrum. BTW: I am not a big fan of +P ammo as it causes excessive wear on a gun. If it is low power loads (mouse fart loads) and you want to shoot the lower pressure and speed ammo you can change you guide spring to a lighter spring to do so. The benefits are less wear on the gun and if you reload you are using less powder.
