Could be the head or head gasket. However, it might be something simpler. The gaskets between the heads and the lower intake are notorious leakers on the 4.0 OHV engines. This allows coolant to be sucked into the runner under manifold vacuum and this can cause misfire. Guess which cylinder is usually affected first .... #4.
Interesting. Did I mention that #4 has a little more compression than #5? It's coming apart, thank you. It should be obvious if that was the issue when I take it off?
