They step down to 5V internally, the one on the right is charger only. The one on the left is an extension to the radio input. All USB ports have a 5V output but the amount of amperage they can put out is different... you wouldn't want to plug a "fan" in to the one on the left. The one on the right is made for quick charging and powering USB devices. I can't see ever needing more than an amp and a half at 5V but if the device requires 2 that plug will power it. I got it from E-bay but just about any place should have them. Make sure both sockets are rated at high amperage some adapters have a wimpy one and a good one if 2. Any modern device will only pull what it needs. My wife's CRV has a USB plug under the arm rest (stock), my phone over taxes it and makes it shut down!
